Stratford Villa Post-Acute in Livermore, CA is seeking an Occupational Therapist to provide and direct all aspects of patient care, including evaluation, treatment planning, goal setting, family education and documentation, in a Five-Star, Medicare-certified skilled nursing facility.

Open to Full-Time, Part-Time, and PRN; new grads welcome. California OT license and BLS required; SNF/LTC experience is a plus. Medical, dental, vision, retirement, PTO, and training opportunities are offered.

Qualifications

• California license to practice Occupational Therapy required.
• Newly licensed OT welcome.
• BLS certification required.
• Experience in Skilled Nursing or long-term care a plus.

Responsibilities

• Conduct comprehensive assessments to identify patients' strengths, limitations, and goals.
• Develop individualized treatment plans based on the patient's needs and goals.
• Provide therapeutic interventions to improve patients' physical, cognitive, and emotional well-being.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Educate patients and their families on adaptive techniques and strategies for independent living.
• Document patients' progress and update treatment plans as needed.
• Stay current with advances in occupational therapy by attending professional development opportunities.

Position Overview:

The Occupational Therapist (OT) role provides and directs all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to organizations guidelines, professional standards and community needs. Serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Participates in quality assurance and customer service activities.

Qualifications:

• Newly licensed OT/new OT graduates are welcome.
• Skilled Nursing or Long-Term Care experience is a plus.
• Previous experience as an Occupational Therapist (OT) is a plus.
• A valid and active California license to practice Occupational Therapy is required.
• BLS Certifications are required.
