Job summary

Apploi is seeking a compassionate Occupational Therapist (OT) to join our Skilled Nursing and Rehabilitation Center in Volusia County, FL. This full-time role offers an hourly pay rate of $48–$58 and requires an active Florida OT license and at least one year of SNF experience.

You will evaluate residents, develop individualized treatment plans, and work with an interdisciplinary team to promote independence, safety, and daily function. Join us to support residents' recovery and quality of life.

Qualifications

• Active Florida Occupational Therapist license.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans based on their needs and goals.
• Provide occupational therapy services that support independence, safety, and participation in daily activities.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and other interdisciplinary team members.
• Participate in resident assessments, care plan meetings, and discharge planning.
• Educate residents and families on strategies to support functional progress and a safe transition after discharge.
• Maintain accurate and timely evaluations, treatment notes, progress reports, and billing documentation.
• Follow established therapy standards, facility policies, and applicable regulatory requirements.
• Support the Rehabilitation Director with clinical initiatives and departmental improvements.
• Participate in quality assurance and performance improvement activities.
• Maintain a compassionate, supportive, and resident-centered therapy environment.
