Job summary

Delmar Center for Rehabilitation and Nursing is seeking an Occupational Therapist (OT) to join our skilled nursing team in New York. You will evaluate residents, develop individualized treatment plans, and deliver therapy to improve daily function and independence.

Collaborate with an interdisciplinary team, participate in care planning, complete timely documentation, and ensure care meets regulatory standards in a friendly, resident‑centered environment.

Responsibilities

• Conduct comprehensive occupational therapy evaluations to assess residents’ functional status, including ADLs, IADLs, cognition, mobility, strength, coordination, balance, and safety awareness
• Develop, implement, and update individualized treatment plans based on physician orders, resident goals, and evidence-based practice
• Provide skilled therapeutic interventions to improve independence in self-care, transfers, positioning, upper extremity function, and adaptive techniques
• Address cognitive, perceptual, and psychosocial deficits impacting daily function and quality of life
• Recommend, train, and educate residents in the use of adaptive equipment, assistive devices, and environmental modifications
• Collaborate closely with an interdisciplinary team (PT, SLP, nursing, social services, and physicians) to coordinate care and support optimal outcomes
• Participate in care plan meetings, discharge planning, and family education to ensure safe transitions and continuity of care
• Monitor resident progress, modify treatment plans as needed, and ensure services meet medical necessity requirements
• Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge summaries in accordance with Medicare, state, and facility guidelines
• Maintain compliance with all regulatory, ethical, and professional standards of occupational therapy practice
• Supervise and collaborate with COTAs and therapy aides as applicable
• Promote resident safety, dignity, and quality of life in a skilled nursing environment
