Job summary

Woodside Health & Rehab seeks a compassionate Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team on a per diem basis. The OT evaluates residents, develops treatment plans, and provides therapy to promote independence, safety, and quality of life.

Responsibilities include ADL-focused therapy, functional mobility, documentation in Medicare guidelines, and collaboration with the interdisciplinary team.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Geriatric rehabilitation knowledge and Medicare regulations.
• Strong communication and documentation skills.

• Provide therapy focused on ADLs, mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.

We are seeking a compassionate and licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides occupational therapy services to promote independence, safety, and improved quality of life.
