Job summary

Adaptive Home Health, LLC is seeking an Occupational Therapist (OT) for field-based, full-time work in Texas. You will complete in-home OT evaluations and deliver skilled visits, focusing on ADLs/IADLs, safety and functional independence.

We support you with an operations-centric team, a productive per-point model, and technology to reduce paperwork. Travel across the assigned territory is expected, and collaboration with RN/MD is essential.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Responsibilities

• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field-based

• Comp: $75.00/point $100-$130,000/year

Requirements

Must-haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfort escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
