Job summary

PASCO/SW Home Health in Colorado is seeking an Occupational Therapist to develop and implement individualized care plans and provide therapy services in home health settings.

You will assess functional status, teach new skills, and collaborate with a caring team to help patients regain independence, with a focus on safety and quality of life. Licensed OT with Colorado credentials and NBCOT eligibility is required.

Qualifications

• Licensed Occupational Therapist in Colorado.
• Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification.
• Minimum 2 years’ clinical experience (home health preferred).
• Current CPR certification; valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services per physician’s orders.
• Assess functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in daily activities.
• Maintain accurate documentation and participate in case conferences.
