Job summary

The Pennant Group, Inc. is seeking a licensed Physical Therapist to join PASCO/SW Home Health in Grand Junction, CO. You will administer home-based PT under the physician’s plan of care, tailor care plans to individual patients, and work with a supportive team to improve mobility and independence.

Requirements include CO licensure, CAPTE-accredited degree, CPR, and at least 2 years of home health experience. A valid driver’s license and reliable transportation are needed.

• Degree from a CAPTE-accredited program; passed state exam.
• Minimum 2 years’ home health experience preferred.
• Current CPR certification.

Responsibilities
