Job summary

The Pennant Group, Inc. is seeking a licensed Occupational Therapist to join PASCO/SW Home Health in Colorado.

You will create personalized treatment plans, teach adaptive skills, and collaborate with a caring team in a home health setting, with a flexible per-visit schedule. Requirements include CO OT license, ACOTE-accredited program, NBCOT eligibility, 2 years’ home health experience, CPR, valid driver’s license and reliable transportation.

Qualifications

• Licensed Occupational Therapist in the State of Colorado.
• Graduate of ACOTE-accredited OT program, eligible for NBCOT certification.
• Minimum 2 years’ clinical experience (home health preferred).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services per physician’s orders.
• Assess functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in daily activities.
• Maintain accurate documentation and participate in case conferences.
