Job summary

Sweetwater Care and Rehab is seeking an Occupational Therapist to join our in-house therapy team. The role involves providing rehabilitative therapy to patients, following regulatory standards, and documenting outcomes to support recovery and functional improvement.

Responsibilities include screening, planning care, supervising rehab staff, and collaborating with the IDT to ensure high-quality patient care. Active CA licensure and NBCOT are required, with state-of-the-art documentation practices.

Qualifications

• Active state licensure in California.
• NBCOT certification or eligibility required.
• Experience supervising rehab staff is preferred.

Responsibilities

• Provide rehabilitative therapy for cognitive and/or ADL disorders.
• Document treatment plans, progress notes and discharge summaries.
• Screen and evaluate patients and develop care plans per standards.
• Supervise and support physical therapy assistants and rehab aides.
• Collaborate with the IDT and communicate patient progress to the team.
• Ensure regulatory compliance and accurate clinical documentation.

Principal Responsibilities

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

• Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
• Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapist.
• Adherence to Company required policy and procedures and code of conduct.
