Job summary

Spyglass Healthcare is seeking a licensed Occupational Therapist in California to deliver high-quality individualized therapy within our rehabilitation teams. You will assess residents, create tailored plans, and drive functional outcomes in a collaborative, patient-centered environment.

Qualifications

• Active California OT license in good standing.
• Bachelor’s or Master’s in Occupational Therapy.
• New graduates are welcome; SNF or rehabilitation experience preferred.
• Knowledge of PDPM, Medicare, and SNF facility regulations.
• Strong communication, organizational, and teamwork skills.
• Commitment to resident-centered, high-quality care.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Implement evidence-based interventions to support functional independence.
• Document resident progress accurately and timely in accordance with regulatory requirements.
• Collaborate with interdisciplinary teams to optimize resident care.
• Participate in care conferences and family meetings, as applicable.
• Mentor Occupational Therapy Assistants and support staff, as applicable.
