Job summary

Spyglass Healthcare is seeking a compassionate Occupational Therapist to provide resident-centered therapy in our California facilities. You will evaluate residents, design individualized treatment plans, and implement evidence-based interventions to maximize independence and safety.

New graduates are welcome; SNF or rehab experience is preferred. You will collaborate with a multidisciplinary team, document progress per regulations, and participate in care conferences.

Qualifications

• Active California OT license in good standing.
• Bachelor’s or master’s degree in occupational therapy.
• New graduates are welcome; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM, Medicare, and skilled nursing facility regulations.
• Strong communication, organizational, and teamwork skills.
• Commitment to resident-centered, high-quality care.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Implement evidence-based interventions to support functional independence.
• Document resident progress accurately and timely per regulatory requirements.
• Collaborate with interdisciplinary teams to optimize resident care.
• Participate in care conferences and family meetings as applicable.
• Mentor Occupational Therapy Assistants and support staff.
