Job summary

Homeside Rehab is seeking licensed Occupational Therapists (OTs) and Occupational Therapist Assistants (OTAs) to provide home-based therapy in Brooklyn, NY, specifically within the 11219 and 11204 ZIP codes. This is a flexible local home-care role with a supportive clinical and administrative team.

Responsibilities include evaluating patient needs, developing and implementing treatment plans, documenting progress, educating patients and caregivers, and coordinating with the clinical team.

Qualifications

• Active New York State OT or OTA license.
• Minimum 1 year of experience preferred.
• Home care experience preferred but not required.
• Reliable transportation.
• Strong communication and documentation skills.
• Ability to work independently in a home care setting.
• Dependable and able to maintain a consistent patient visit schedule.

• Provide skilled therapy services in the patient's home.
