Homeside Rehab is actively seeking licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to deliver home-based therapy to patients in Borough Park, Brooklyn, within ZIP codes 11219 and 11204. This role offers flexible, local cases and a supportive clinical/administrative team.

Candidates should hold NY PT or PTA licensure, have at least 1 year of experience, and possess strong communication and documentation skills.

Qualifications

• Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license.
• Minimum 1 year of experience preferred.
• Home care experience preferred, but not required.
• Reliable transportation.
• Strong communication and documentation skills.
• Ability to work independently in a home care setting.
• Dependable and able to maintain a consistent patient visit schedule.

Responsibilities

• Provide skilled physical therapy services in the patient's home.
• Evaluate patient needs and develop appropriate treatment plans.
• Provide treatment according to the established plan of care.
• PTAs will provide treatment under the supervision of a licensed Physical Therapist.
• Monitor patient progress and communicate changes to the clinical team.
• Educate patients and caregivers regarding treatment plans and home exercise programs.
• Complete accurate and timely clinical documentation.
• Maintain professional communication with patients, caregivers, and the Homeside Rehab team.

Homeside Rehab is seeking licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide home-based therapy services to patients in Borough Park, Brooklyn, specifically within the 11219 and 11204 ZIP codes.
