Evolve Therapy Services in New Haven, CT is hiring a dedicated Occupational Therapist for an in-house Skilled Nursing Facility. This full-time or PRN role focuses on evaluating residents, planning individualized therapy, and collaborating with an interdisciplinary team to maintain high-quality resident care.

Candidates must hold a Degree in Occupational Therapy and an active OT license in the state. Ongoing professional development is required.

Qualificações

Responsabilidades

• Conduct regular screenings to determine best intervention for each resident.
• Evaluate residents to inform treatment plans with short- and long-term goals.
• Communicate evaluation findings and plans to residents, families, and team members.
• Establish individual therapy schedules with the care team.
• Provide therapy per plan and ensure proper documentation and billing.
• Perform duties to treat patients and support clinical excellence.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
