Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. Multiple schedules available: Full Time, Part Time, or PRN.

The role involves planning, directing OT services, and ensuring high-quality resident care in accordance with applicable standards. CMS documentation and collaboration with the interdisciplinary team are essential.

Qualifications

• Associate's degree in Occupational Therapy or equivalent.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans to residents; follow short- and long-term goals.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of therapies.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice with daily point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
