Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Tupper Lake, NY. This full-time, part-time, or PRN position includes a sign-on bonus and opportunities to grow within the organization.

The OT will plan, organize, develop, and direct therapy services, coordinate with the interdisciplinary team, ensure compliant documentation and billing, and supervise COTA as needed to maintain high-quality resident care.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the established treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure/certification

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time (Sign On Bonus!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
