Job summary

Source Code in Modesto, CA is seeking an Occupational Therapist to coordinate restorative therapy, support ADLs, and help participants regain independence. This full-time role emphasizes collaborative care with Physicians, rehab staff, and the IDT.

You will conduct assessments, participate in care planning, document interventions, and guide Therapy Assistants in Functional Maintenance Programs, with a focus on work-life balance and lower patient volumes.

Qualifications

• Bachelor's degree in occupational therapy; master's preferred.
• Valid California Occupational Therapy License, or proof of license eligibility.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Responsibilities

• Conduct Occupational Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs
• Work with Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)

Occupational Therapist

Modesto, California

Full-Time

The Occupational Therapist coordinates and provides restorative and rehabilitative occupational therapy services, working closely with the Physician, rehabilitation staff, and other IDT members to maximize participant independence and safety, as well as enhance performance of ADLs.

This role is different because Occupational Therapists at our facility:

• Lower patient volumes - no more seeing 3-4 patients an hour


• Form long-term relationships with participants rather than short-term rehab stints


• Collaborate with other therapists, providers, and disciplines


• Have work/life balance - no after-hours documentation

On the day-to-day, you will:
