Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Clearfield, PA. This role supports residents through screening, evaluation, and therapy planning within a collaborative team.

Responsibilities include direct patient therapy, planning schedules, and ensuring accurate documentation and billing. A valid OT license and ongoing education are required, with growth opportunities within the organization.

Qualifications

• Must have a Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with the interdisciplinary team.
• Conduct therapy per established plan and ensure proper documentation and billing of therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement standards.
