Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Ahoskie, NC. The role supports therapists, plans therapy services, and ensures quality resident care in a CMS-guided setting.

The OTA will screen residents, implement treatment plans, document care, and collaborate with an interdisciplinary team to maximize patient outcomes. Licensure and ongoing CE are required.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Implement treatment plans following the supervising therapist's guidance.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual therapy schedules with team members.
• Document and bill occupational therapy services as per plan of care.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards and daily coding.

Education
