Job summary

Responsibilities include documenting care, supervising staff and students, contributing to performance improvement initiatives, and participating on hospital committees to improve health outcomes. Discharge planning and patient education are emphasized.

Qualifications

• Graduate of an accredited OT program.
• OT licensure or eligibility within state requirements.
• Basic computer literacy to document care and coordinate with team.

Responsibilities

• Evaluates and assesses the patient based on subjective and objective information.
• Develops a written plan of care.
• Provides treatment according to established goals on plan of care.
• Documents interventions/care in the patient's medical record.
• Evaluates effectiveness of plan of care and reassesses functional status.
• Discharge planning information including education, follow-up, and referrals to community resources.
• Contributes to staff and student development.
• Participates in activities designed to improve health care delivery.
• Completes assigned goals.
