Job summary

Metro Physical & Aquatic Therapy is seeking a Full-Time Occupational Therapist to join our Homecare team in the Riverhead area. You will provide 1:1 occupational therapy in patients’ homes, evaluate needs, and develop individualized treatment plans in coordination with physician orders.

Qualifications

• OT program graduate is required.
• Licensed OT in NY or eligible.
• Home health experience preferred but not required; mentor provided for documentation/compliance.
• Ability to travel to patients’ homes within the territory.
• Strong communication with patients, caregivers and management.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards and the plan of care.
• Educate family members and caregivers on proper therapy techniques.

• Perform and document an initial exam and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment and adjust as needed.
• Treat geriatric patients.

What You’ll Do

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Additional Information

The anticipated base salary range for this position is $90,000.00-$100,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
