Job summary

PruittHealth @ Home is seeking an Occupational Therapist for Georgia-Blue Ridge home health to evaluate, consult, and treat patients referred by physicians in their residences. 2 full-time openings with a pay model that ties compensation to effort and visits, plus opportunities for growth and flexible schedules.

Responsibilities include assessments, care planning with the interdisciplinary team, and timely documentation (OASIS, SOC, ABNs).

Qualifications

• Home Health experience is preferable.
• Must have a valid, unrestricted professional license in state of practice.
• Graduate of ACOTE accredited program; if foreign-trained, credentials evaluated for US equivalence.

Responsibilities

• Assess and monitor patient health, document progress using required forms and technology.
• Collaborate with interdisciplinary team to develop and manage the plan of care.
• Demonstrate competency in home-health patient care and prioritize needs.
• Ensure timely, accurate documentation (OASIS, SOC/Admissions, recertifications, ABNs).
• Maintain current licenses, certifications, and patient records per regulations.
• Support training/orientation of other staff and participate in required program activities.
