Job summary

Your Health Organization seeks an Occupational Therapist to serve patients across Gainesville, with visits to homes, ALFs, and ILFs. This full-time role offers a salary-based structure with a monthly bonus and a standard workday from 8:00 AM to 5:00 PM, Monday through Friday.

The OT will assess, plan, and supervise therapy, coordinating with COTAs and regional leadership to optimize patient outcomes while ensuring compliance with regulatory requirements.

Qualifications

• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Current state license to practice as an Occupational Therapist.
• Minimum of two years of clinical experience, preferably in home health or outpatient setting.
• Proficiency in Telehealth platforms and EHR.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Conduct comprehensive evaluations of patients' physical, emotional, and social conditions.
• Identify patient needs and set realistic goals for therapy.
• Develop individualized treatment plans aimed at improving daily living and working skills.
• Supervise and guide COTAs; review and monitor their implementation of plans.
• Use telehealth tools to collaborate with COTAs and other healthcare professionals.
• Document care plans and outcomes to maintain continuity of care.
• Coordinate with care team and providers to align on treatment plans.
