Your Health is seeking a skilled Primary Care Occupational Therapist to service patients across the Gainesville region. The role includes home, ALF, and ILF visits, collaboration with COTAs, and developing individualized treatment plans to restore independence and daily living skills.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy from an accredited program.
• Current state license to practice as an Occupational Therapist in good standing.
• Minimum of two (2) years of clinical experience, preferably in a home health or outpatient setting.
• Proficiency in Telehealth platforms and electronic health records (EHR).
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsabilités

• Conduct comprehensive evaluations of patients’ physical, emotional, and social conditions.
• Identify patient needs and set realistic goals for therapy.
• Develop individualized treatment plans aimed at improving patient’s daily living and working skills.
• Adapt treatment plans based on patient progress and feedback.
• Work in conjunction with COTAs, providing guidance and supervision.
• Review and monitor the implementation of treatment plans by COTAs.
• Use telehealth tools to collaborate with COTAs and other healthcare professionals.
• Participate in care team meetings as necessary.
• Communicate effectively with management regarding patient outcomes and program improvements.
• Facilitate daily visits with providers to ensure timely communication and coordination of care.
