Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to a dependable vehicle or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy.
• Evaluates patient functional status and documents CMS-relevant data.
• Observes, records and reports patient responses to treatment and changes in condition.
• Conducts patient assessments of functional ability.
• Adaptations and home assessments as defined by plan of care.
