Job summary

UnitedHealth Group is seeking an Occupational Therapist for Home Health to assess and address patient care needs related to functional status, ADLs, and home safety, integrating equipment and adaptive strategies into care plans.

The role requires licensed OT in the practice state, a valid driver's license, and reliable transportation, with preferred CPR certification and strong communication and organizational skills. This is a full-time position supporting home-based care teams.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status and CMS documentation needs; consults with the physician in the plan of care.
• Observes, records, and reports to the nurse supervisor and physician the patient's response to treatment and condition changes.
• Conducts patient assessments evaluating functional level using diagnostic and prognostic functional ability tests.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
