Job description

Job Opportunity: Occupational Therapist (OT) at BAYADA Home Health Care

Location: Williston, VT (South Burlington Visits Office)

Position: Full-time, home health visits for adult and geriatric patients in Washington County.

Responsibilities:

1. Conduct home visits and evaluate patients' functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
2. Assess cognitive, psychological, and motor performance; develop and revise individualized intervention plans.
3. Evaluate home environment for hazards; recommend equipment needs and environmental modifications.
4. Collaborate with the multidisciplinary team and document patient care using electronic medical records.
5. Educate patients and families on rehabilitative activities to promote health, safety, and independence.

Qualifications:

1. Current Vermont Occupational Therapist license.
2. Minimum one year of recent OT experience; home care experience preferred.
3. Graduation from an accredited occupational therapy program.
4. Ability to work independently, manage time effectively, and possess strong interpersonal and organizational skills.
5. Proficiency with computers; EMR experience preferred.
