Key Responsibilities

• Conduct Occupational Therapy Evaluations: Assess patients' physical, cognitive, and psychosocial function to design individualized care plans that restore independence.
• Implement Evidence-Based Interventions: Facilitate therapeutic activities to improve ADLs (Activities of Daily Living), range of motion, fine motor skills, and functional mobility.
• Promote Patient Safety & Independence: Instruct patients in the use of adaptive equipment, home modifications, and techniques for safer mobility and self-care.
• Collaborate with Healthcare Providers: Work closely with physicians, nurses, and family caregivers to ensure aligned, holistic rehabilitation goals.
• Monitor Progress & Adjust Treatment: Document progress in EMR, modify plans of care as needed, and provide measurable outcomes in accordance with Medicare and home health standards.
• Educate Families & Caregivers: Offer training and resources to family members to support ongoing occupational therapy exercises and home-based recovery.

Qualifications

• Current State Licensure as an Occupational Therapist (OT)
• Certified in CPR and BLS, with active status
• Minimum 1 year of occupational therapy experience in a clinical, outpatient, or home health setting (strong new grads are encouraged to apply)
• Geriatric rehabilitation experience preferred
• Strong communication, time management, and documentation skills
• Proficient with electronic medical records (EMR) systems
• Must have a valid driver’s license and reliable transportation for in-home therapy visits
