Job summary

CommonSpirit Health at Home is seeking a compassionate Occupational Therapist for a PRN home health position in Bardstown, KY. The OT will evaluate patients, set goals, and develop plans to promote independence and improve quality of life in the home setting.

The role requires an active OT license, CPR/BLS certification, and at least 1 year of OT experience in a clinical environment, along with a valid driver's license.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Active registered OT license in the state where you practice.
• CPR/BLS certification required.
• One year of Occupational Therapy experience in a clinical setting preferred.
• Current driver's license.

Responsibilities

• Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs; develop and implement personalized treatment plans including exercises, adaptive techniques, and assistive devices to improve functional abilities.
• Teach patients and families about home safety, adaptive equipment, and strategies to manage daily activities.
• Maintain accurate and timely records of patient progress, treatment plans, and program modifications for compliance and regulatory standards.
• Monitor patient progress and adjust treatment plans as necessary; use outcome-based measures to assess effectiveness of therapy interventions.
• Collaborate and communicate changes in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

This is a PRN Position

We are seeking a skilled and compassionate Occupational Therapist to join VNA Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities

• Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises', adaptive techniques, and use of assistive devices to improve patients functional abilities.


• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.


• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.


• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.


• Collaborate and communicate patients' change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
