Responsibilities

• Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative treatment intervention to patients with cognitive and/or ADL physical functioning disorders.
• Administers various procedures as part of the rehabilitation plan.
• Assesses patient’s progress and adjusts treatments accordingly.
• Evaluates treatment outcomes for effectiveness.
• Provides consultation and counseling to patients and families.
• Effectively communicates with supervisor and health team members regarding patient progress.
• Provides comprehensive discharge summary of services provided.
