Responsibilities

• Perform diagnostic and screening procedures within scope of practice
• Respond to patient needs with compassion and clinical judgment
• Provide culturally sensitive care to a diverse patient population
• Manage a full-time panel of chronic patients, calling between visits when TNCC Certification numbers drift
• Coordinate discharge planning alongside case management, social work, and the patient's family in Gresham
• Run point on Ventilator Management education days, translating clinical guidance into language patients keep
