Job summary

Home & Health Care Management, an established Northern California home health care agency, seeks an Occupational Therapist to provide in-home therapy across our service area. You will treat older adults, children, and individuals with disabilities, traveling to patients’ homes within Butte, Shasta, Yuba-Sutter counties and surrounding communities.

The role offers full-time 40 hours/week or part-time 10–20 hours/week, with potential for expanded hours.

Qualifications

• One year of experience as a home health care provider.
• Current California State license as an Occupational Therapist and ongoing education.
• Current CPR certification.
• Valid California Driver’s License.
• Able to be insured under the liability policy with a clean DMV report.
• Own vehicle with current auto insurance.
• COVID vaccination or exemption; PPE provided.

Responsibilities

• Evaluate patients upon physician diagnosis.
• Assess patients to identify deficits in self-care, mobility, cognition and daily life activities.
• Deliver occupational therapy in patients’ homes.
• Improve motor skills, coordination and balance through therapy exercises.
• Develop or regain daily living skills (dressing, bathing, eating).
• Instruct use of adaptive equipment to enhance independence.
• Collaborate with physicians, nurses, social workers, physical and speech therapists to coordinate care.
• Monitor and document patients’ progress during therapy sessions.

Job description

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Who You Are:

You are a motivated Occupational Therapist who shares our vision of delivering in-home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment.

Travel

This role comprises 100% travel within our service region. You will be traveling to patients’ homes.

Service Region:

We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding communities.

Schedule

We are hiring for Full-Time 40 hours per week M-F 8am-5pm or Part-Time/Per-Diem role of between 10-20 hours per week, with the possibility of expanded hours in the future.

Duties and Responsibilities:

• Evaluate patients upon physician diagnosis
• Assess patient and identify deficits impacting self-care, functional mobility, behavior, cognition and patient’s ability to function in their life’s role
• Deliver occupational therapy to patients in their own homes
• Help patients improve their motor skills, coordination, and balance through therapeutic exercises and activities
• Assist patients in developing or regaining daily living skills, such as dressing, bathing, and eating
• Teach patients how to use adaptive equipment and assistive devices to enhance their independence
• Collaborate with other healthcare professionals, such as physicians, nurses, social workers, physical therapists and speech therapists, to coordinate patient care
• Monitor and document patients' progress during therapy sessions

Essential Requirements:

• One year experience as a home health care provider
• Current California State license as an Occupational Therapist and stays updated through independent study and continuing education
• Current CPR certification
• Valid California Driver’s License
• Eligible to be insured under our liability policy with a clean DMV report
• Your own vehicle and proof of current auto insurance
• COVID vaccinated or have an exemption. PPE is provided to all employees

Physical Requirements:

Conditional upon job offer is successfully passing a medical exam. Physical requirements for this role specifically include

• Sits, stands, bends, lifts, walks, and moves intermittently during working hours.
• Able to lift to 50 pounds without assistance.
• Able to drive intermittently on a daily basis

Compensation:

Pay range is $55.00 to $65.00 per hour. Exact compensation may vary based on skills, experience, and location.
