Position Overview

An Occupational Therapist in home health provides personalized, one-on-one care to help patients live independently and safely in their homes. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, and interpreting test results to set functional goals. The therapist develops and documents plans of care, monitors progress, and adjusts treatments as needed. They ensure compliance with physician orders, collaborate with healthcare teams, and educate patients and caregivers on home exercise plans. The role involves supervising other personnel such as OT Assistants and Home Health Aides, completing assessments per state regulations, and maintaining thorough documentation to prevent claim denials. Strong communication skills are essential to coordinate with families and team members.

Qualifications & Skills

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license (WI and MN)
• Valid CPR certification and driver's license
