Job summary

Cherry Hill Free Clinic is seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions in a home health setting according to physician directives. You will conduct assessments, develop care plans, and provide therapy services, documenting progress and collaborating with the care team.

Ideal candidates hold a bachelor's in OT, an unrestricted state license, CPR and a valid driver's license with insurance. Home health and OASIS experience are preferred.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Current, unrestricted state OT license.
• CPR certification and valid driver's license with proof of automobile insurance.
• Experience in home health therapy and OASIS documentation preferred.

Responsibilities

• Assess patients' mobility, strength, and function and develop plans of care.
• Deliver OT services as ordered by physicians and monitor progress.
• Document patient progress and adjust treatment as needed.
• Collaborate with healthcare team, patients, and caregivers.
• Participate in discharge planning and education; supervise OT Assistants as applicable.

Position Overview:

Key Responsibilities:

• Conduct initial and ongoing assessments of patients' mobility, strength, and function
• Develop and update individualized plans of care
• Provide therapy services in accordance with physician orders
• Document patient progress and modify treatment as needed
• Collaborate with healthcare team members, patients, and caregivers
• Participate in discharge planning and education
• Supervise other personnel such as OT Assistants and Home Health Aides where applicable
• Complete OASIS assessments where permitted

Qualifications & Skills:

• Bachelor's or higher degree in Occupational Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR, driver's license, and proof of automobile insurance
• Experience in home health therapy and OASIS documentation preferred
