Job summary

Patriot At Home in Dayton, Ohio, is seeking a full-time Occupational Therapist to cover a 30 mile radius. The role involves evaluating patients at home, performing comprehensive assessments (including OASIS), and coordinating with physicians to ensure appropriate orders.

You will educate families, supervise care, and maintain regular communication with medical staff. We offer flexible scheduling, paid time off, minimal holiday rotation, hardship support, and a 401K with company matching.

Qualifications

• Must be a graduate of an Occupational Therapy school approved by the AMA/AOTA.
• Preferably a member of the American Occupational Therapy Association.
• Must be licensed by the state.
• Must have at least one year of institutional experience.
• Must have a criminal background check.
• Must have current CPR (BLS) certification.

Responsibilities

• Assist the physician in evaluating the patient's level of function by applying diagnostic and prognostic procedures.
• Conduct initial and ongoing comprehensive assessments, including OASIS assessments.
• Assure that the physician's orders are appropriate and discuss necessary changes.
• Teach, supervise, and counsel the family and patient in the total Occupational Therapy program at home.
• Communicate with the physician on a regular basis regarding patient's status and orders.

• Conduct initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments

• Assure that the physician's orders are appropriate and discusses necessary changes

• Teach, supervise, and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home

• Communicate with the physician on a regular basis regarding patient's status, obtaining additional orders, if necessary

Qualifications:

• Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association

• Preferably should be a member of the American Occupational Therapy Association

• Must be licensed by the state

• Must have at least one year of institutional experience

• Must have a criminal background check

• Must have current CPR (BLS) certification
  

Job Conditions:

• The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required

• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care

• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds
