Job summary

Patriot At Home in Ohio is seeking a full-time Occupational Therapist to provide in-home therapy within a 30-mile radius of Fairfield. You will evaluate patients, develop individualized plans, and collaborate with physicians to maximize independence.

The role requires OT licensure, active CPR, and at least one year of institutional experience, with preferred AOTA membership. You will work in a compassionate home health setting with flexible scheduling and competitive compensation.

Qualifications

• Must be a graduate of an Occupational Therapy school approved by the AMA/ AOTA.
• Preferably a member of the American Occupational Therapy Association.
• Must be licensed by the state.
• Must have at least one year of institutional experience.
• Must have a criminal background check.
• Must have current CPR (BLS) certification.

Responsibilities

• Assist the physician in evaluating the patient's level of function.
• Conduct initial and ongoing comprehensive assessments including OASIS.
• Ensure that physician orders are appropriate and discuss changes as needed.
• Teach, supervise, and counsel the family and patient in the home OT program.
• Communicate with the physician regularly regarding patient status and orders.

The responsibilities of our Occupational Therapist include:

• Assist the physician in evaluating the patient's level of function by applying diagnostic and prognostic procedures
• Conduct initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments
• Assure that the physician's orders are appropriate and discusses necessary changes
• Teach, supervise, and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home
• Communicate with the physician on a regular basis regarding patient’s status, obtaining additional orders, if necessary

Qualifications:

• Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association
• Preferably should be a member of the American Occupational Therapy Association
• Must be licensed by the state
• Must have at least one year of institutional experience
• Must have a criminal background check
• Must have current CPR (BLS) certification

Job Conditions:

• The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds
