Renewal Rehab, part of Enhance Therapies, is seeking a licensed Occupational Therapist for a full-time role in a skilled nursing facility setting. You will evaluate, treat, and develop care plans within a collaborative team, ensuring regulatory compliance and high-quality patient outcomes.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification required or as state requires.
• Active OT license in the practicing state.

Responsibilities

• Evaluate and treat patients with functional and cognitive impairments and ADLs.
• Create and implement individualized care plans.
• Collaborate with interdisciplinary team to ensure quality outcomes.
• Maintain accurate documentation to meet regulatory requirements.
• Obtain and update all physician orders as required.

What You'll Do

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meet all regulatory requirements
• Ensure all required physician orders are obtained and up to date

Qualifications

• Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program
• Certified by the NBCOT as an OTR, if required by the state
• Current license as an Occupational Therapist in the state of practice
• Strong communication and organizational skills
