Job summary

Renewal Rehab is seeking a licensed Occupational Therapist for a full-time role in a skilled nursing facility/long-term care setting. You will evaluate and treat patients, create personalized care plans, and collaborate with a multidisciplinary team.

Qualifications

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure physician orders are obtained and up to date.
