Job summary

Renewal Rehab is seeking a licensed Occupational Therapist for a full-time role in a Skilled Nursing Facility / Long-Term Care setting in Illinois. You will evaluate and treat patients, develop personalized care plans, and collaborate with a multidisciplinary team to ensure quality outcomes.

Qualifications

• Must have an OT degree from an AOTA‑accredited program.
• Current license as an Occupational Therapist in the state of practice.
• NBCOT credential as OTR, if required by state regulations.
• Strong communication and organizational skills; new grads welcome with mentorship.
• Immigration support for H1B candidates available.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure physician orders are current and obtained.

Qualifications
