Job summary

Renewal Rehab, part of Enhance Therapies, is seeking an Occupational Therapist for a full-time role in a skilled nursing facility / long-term care setting in Illinois. You will evaluate and treat patients, develop personalized care plans, and work with a collaborative team to drive quality outcomes.

Qualifications

• Must hold an OT degree from an AOTA-accredited program.
• NBCOT certification as an OTR if required by state.
• Current OT license in the state of practice.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meet all regulatory requirements
• Ensure all required physician orders are obtained and up to date
