Job summary

Qualifications

Responsibilities

• Performs evaluations, determines OT diagnoses and develops plan of care.
• Reassesses including OASIS and updates plan of care with physician input.
• Provides therapeutic treatments and equipment evaluation; prepares progress notes.
• Builds collaborative relationships with multi-disciplinary teams for patient care.
• Coordinates supervision of OTAs and home health aides; supports team training.
• Modifies plans to maximize therapeutic benefit and return to discharge criteria
