Job summary

Concept Rehab, Inc. seeks a dedicated Occupational Therapist to join our therapy team at a Skilled Nursing facility serving Kettering and Centerville, Ohio.

The two facilities are 3 miles apart, offering a flexible full-time opportunity to help patients maximize independence and functional outcomes. You will evaluate patients, develop individualized plans, supervise OTAs, collaborate with physicians and families, participate in care planning, and ensure accurate documentation and billing in a

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Occupational Therapy.
• Current Occupational Therapist license in the state of practice.
• Strong communication and interpersonal skills.
• Ability to work independently and within a collaborative healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled occupational therapy services.
• Help patients improve daily living skills, mobility, cognition, strength, and independence.
• Monitor progress and adjust treatment plans as needed.
• Supervise Occupational Therapy Assistants (OTAs).
• Collaborate with physicians, nurses, families, and interdisciplinary team members.
• Participate in care plan and discharge planning meetings.
• Educate patients, caregivers, and facility staff.
• Recommend equipment and interventions to improve patient outcomes.
• Maintain timely, accurate documentation and billing.
