Job summary

Tier1usa's Renewal Rehab is seeking an Occupational Therapist for a Full-Time position in a Skilled Nursing Facility in Beardstown, IL. A current OT license is required to apply.

You will evaluate and treat patients with functional, cognitive, and ADL impairments and develop personalized care plans. You will collaborate with a multidisciplinary team and ensure documentation complies with regulatory requirements.

Qualifications

• Must have OT degree from AOTA-accredited program
• NBCOT certification as OTR if required by state
• Current OT license in the state of practice
• Strong communication and organizational skills

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team to ensure quality outcomes
• Maintain documentation to meet all regulatory requirements
• Ensure all required physician orders are obtained and up to date
