Job summary

Renewal Rehab is hiring a licensed Occupational Therapist for a full-time on-site role in a Skilled Nursing Facility in Illinois. You will evaluate and treat patients with functional, cognitive, and ADL impairments, and develop personalized care plans tailored to each individual.

You will collaborate with a multidisciplinary team to ensure quality outcomes, maintain complete documentation to regulatory standards, and verify all physician orders are current.

Qualifications

• Must be licensed as an Occupational Therapist to apply.
• NBCOT certification may be required by state.
• Active state license in Illinois or eligible to practice in IL.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
