Renewal Rehab in Mount Carroll, IL is hiring a full-time Occupational Therapist to join a Skilled Nursing Facility team. You will evaluate and treat patients with functional, cognitive, and ADL impairments, develop personalized care plans, and ensure documentation meets regulatory requirements.

Qualifications

• Must hold a degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT certification as an Occupational Therapist may be required by state law.
• Current Illinois OT license to practice in the state.
• Strong communication and organizational skills for collaborative care.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans for each patient.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Obtain and update all required physician orders.
