Life Care Home Health is seeking an Occupational Therapist to provide patient‑centered care in home health settings across Texas and neighboring areas.

You will assess, plan, implement, and adjust therapy interventions for clients of all ages, coordinating with families and interdisciplinary teams, while maintaining compliance with licensure and CPR requirements. The role emphasizes functional independence and safety, with opportunities for CEUs and career growth in a supportive team.

Qualifications

• Two (2) years’ experience as an OT in a clinical setting.
• Home health experience preferred.
• Active OT license in applicable states and CPR current.

Responsibilities

• Perform clinical assessments; develop and revise Plan of Care.
• Document progress notes, POC, and orders timely.
• Assess ROM, strength, and mobility; adapt equipment and environment.
• Communicate effectively with clients, families, and team members.
• Ensure compliance with third‑party payers and regulations.
