You will work with an interdisciplinary team to address physical, cognitive, and emotional needs, focusing on functional abilities and participation in daily activities.

You will guide residents toward independence, document progress, and ensure regulatory compliance across care plans.

Responsibilities

• Educate residents, families, and caregivers on functional activities and strategies to promote independence
• Document resident progress and treatment plans in accordance with regulatory requirements
• Participate in care conferences and develop treatment plans that align with resident goals
