Job summary

Avera Sacred Heart Pavilion is seeking an Occupational Therapist to coordinate and deliver OT services for patients with diverse conditions. You will implement individualized treatment programs, assess progress, and collaborate with a multidisciplinary team to ensure optimal patient outcomes.

The role requires applying age-appropriate care from infants to geriatrics, documenting results, and contributing to program development within the department.

Qualifications

• Active Occupational Therapist license in the state of practice.
• Ability to communicate effectively with patients, families, and care teams.
• Ability to work within a multidisciplinary team and follow care plans.

Responsibilities

• Demonstrates knowledge of age-specific factors and provides care from infant to geriatric as per guidelines.
• Develops and implements individualized OT treatment plans based on assessments.
• Uses activities to promote health and achieve therapy goals.
• Provides direct OT services in various settings and coordinates with rehab and nursing teams.
• Prepares evaluation results, treatment plans, and progress notes.
