Centra Healthcare Solutions is seeking an experienced or newly graduated Occupational Therapist to join a Skilled Nursing Facility in California on a direct hire basis. The OT will deliver skilled services, assess patient needs, and collaborate with the care team to meet goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist (OT) or actively pursuing licensure.
• Current CPR certification.
• Provide skilled OT services per physician orders.
• Assesses patient needs and plans, evaluates and modifies OT care.
• Collaborates with all disciplines to plan and evaluate patient goals.

Responsibilities

• Provide skilled occupational therapy services/interventions per physician orders.
• Assess patient needs and develop, implement, and modify OT plans to meet goals.
• Collaborate with other disciplines to align team goals for each patient.
