Job summary

Bridge View Nursing Home seeks a per diem Occupational Therapist for day shifts in a skilled nursing setting with a collaborative rehab team. The role involves evaluating residents, developing and implementing treatment plans to maximize independence, and documenting progress in daily notes and certifications.

Requirements include graduation from an accredited OT program and a current NY license; experience is welcome but not required.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current New York State Occupational Therapist License.
• Experience is a plus, though not required.

Responsibilities

• Evaluate and develop treatment plans for residents based on their medical diagnoses.
• Implement treatment plans to improve residents' goals toward functional independence.
• Assess and document residents' progress in detailed re-certifications, progress reports, and daily notes.
• Maintain resident confidence by ensuring the confidentiality of all information.
• Maintain documentation of resident care services by auditing resident and department records.
• Supervise occupational therapy assistants.
• Participate in care plan meetings and contribute to related documentation.
• Plan, develop, and organize OT-related reports and data tracking as directed by the Rehab Director.
• Assist in departmental projects as needed.

Education
